This Website is the property of Theralife Clinics North America, Inc. (“Theralife” or “we”).  Theralife is not a healthcare provider, but provides certain non-clinical administrative and business support services to independent medical providers and/or practices (“provider partners”).

We take the privacy of all visitors to this Website very seriously and therefore set out in this privacy policy our position regarding certain privacy matters and the use of cookies and other tracking technologies on this Website.

5.2.4     At your request, we may disclose your personal information to provider partners in order to provide you with a consultation and/or further information about the medical procedure you are interested in.

5.2.5     We may also aggregate and/or de-identify information we collect so that the information does not identify you.  Aggregated and/or de-identified information is not subject to this privacy policy and may be disclosed to third parties in accordance with applicable laws.

We do not “sell” personal information for money, but we engage in routine practices with our services and Website involving third parties that could be considered a “sale” or “sharing” as defined under California law.  The categories of personal information sold, shared or disclosed for a business or commercial purpose in the past 12 months, and to whom, are as follows:  first name, last name, email address, phone number, and zip code.

8.3     Right to Deletion and Correction. You have the right to request deletion of your personal information that we collect, subject to certain exceptions (such as to detect a security incident or other illegal activity). You also have the right to request that we correct inaccurate personal information we maintain about you.

8.4     Right to Opt-Out. You have the right to opt-out of the sale of your personal information to third parties. Theralife does not have actual knowledge that it sells personal information of minors under the age of sixteen (16).

8.5     Right to Non-Discrimination. You have the right to not receive discriminatory treatment if and when you exercise your rights under the CCPA.
